The SSS has issued an updated list of documentary requirements for sickness and disability benefits in support of the digitalization of the SSS benefit process and in compliance with Republic Act No. 11032 or the Ease of Doing Business and Efficient Government Service Delivery Act of 2018.

Cited below are the basic documentary requirements for sickness benefits and disability claims:

I. Sickness Benefits

  1. SSS Medical Certificate; and
  2. Attending physician’s Medical Certificate issued with the following information:

a. Full name,

b. PRC number,

c. Clinic address,

d. Contact information,

e. History of present illness and complete diagnosis, and

f. Recommended number of days of convalescence including recuperation.

II. Disability Claims

  1. Disability Claim Application (DisCA) Form;
  2. Member’s photo and signature; and
  3. Attending physician’s Medical Certificate issued within six months from the date of filing with the following information:

a. Full name,

b. PRC number,

c. Clinic address,

d. Contact information,

e. History of present illness and complete diagnosis.

Additional supporting documents depending on the nature of the sickness or disability will also be requested by SSS. For the complete list of additional supporting documents, please refer to SSS Circular No. 2022-013.
